The Gold Star Plate is available to individuals who are the surviving spouse, parent, child or a sibling of a person who served in the Armed Forces of the United States and lost his or her life.

The following vehicles will be eligible:

• Passenger cars
• Multi-purpose vehicles
• Motorcycles
• Trucks (10,000 lbs. or less)

You must submit:

• Application for Military Related License Plates (form # VR-120)
• Applicable fee

And one of the following:

• Copy of DD-3 approved by the Department of Defense
• Copy of casualty report; or
• An overseas death certificate issued by the military indicating the name and date of death.
